An investigation has been launched after a pregnant woman who was forced to wait two hours for an ambulance later lost her baby.
The woman was awaiting transfer to the Royal Gwent Hospital in Newport, south Wales, for an emergency caesarean section.
The woman was at Ysbyty Ystrad Fawr in Ystrad Mynach, which has a midwife-led baby unit.
She was eventually transferred to the hospital in Newport where her baby died last week during an attempted forceps delivery.
The local health board has confirmed that it and the ambulance service are now jointly investigating the death.
