Patients offered rapid access at home to GPs

GPs from Goole, Howden and the West Wolds area will work with local health workers and council social care staff to provide care closer to home for the vulnerable.

Officials say the venture aims to give people “good quality community care by the right person, in the right place at the right time, whilst ensuring the long-term sustainability of the NHS in the East Riding”.

They hope the development will have a “huge impact” on inappropriate admissions to hospital, lead to shorter lengths of stay for patients who do need hospital care and more patients managing their conditions.

Patients with a complex range of conditions will be given rapid access within two hours to GPs for advice and visits if they need extra support in a service also available out of normal hours.

They will have a named worker to co-ordinate their care and a care plan. If their condition deteriorates they will have checks at home to avoid unnecessary admissions to hospital.

Holme on Spalding Moor GP Ben White said: “The service will be designed in order to best meet the needs of the local population and to make the best use of the existing staff, perhaps working in different ways in slightly different roles.”
