Nahki Wells was the hero as his brace helped City book a return to Wembley in the League Two play-off final at the expense of Burton.

The League Cup finalists, who were beaten 5-0 by Swansea in February, had trailed 3-2 from the first leg.

But Wells capitalised on a defensive error to level the tie for Bradford, in their 63rd game of the season.

James Hanson then gave them an aggregate lead for the first time with a fine finish after good work by Wells.

2. 2010-11 Huddersfield Town 3 Bournemouth 3 - AET 4-4 Town 4-2 win on pens.

Huddersfield secured a League One play-off final place after a thrilling 4-2 win on penalties following a pulsating tie which ended deadlocked at 3-3 after extra-time.

Lee Peltier, moved into midfield, put the hosts - who drew 1-1 in Dorset - in front in the 26th minute following a corner from set-piece specialist Gary Roberts.

But Steve Lovell’s penalty levelled matters after Adam Smith was felled in the box, with Lovell faring better than Danny Ings did in the first leg, when Ian Bennett saved the forward’s spot-kick.

But Town regained the lead at the interval, through a fierce left-foot shot from Danny Ward moments before the break.

The Cherries equalised for a second time on 63 minutes when Lovell netted after good work by Donal McDermott.

Extra time ensued and substitutes Michael Symes and Marc Pugh combined to set up Ings to put the visitors ahead for the first time on the night, but Antony Kay soon levelled following Roberts corner.

Jason Pearce was dismissed late on ahead of penalties, with misses from Liam Feeney and Anton Robinson proving pivotal as Town triumphed.

3. 2007-08 Carlisle United 0 Leeds United 2, Leeds win 3-2 on aggregate.

Teenager Jonathan Howson was the Whites hero, firing home a sweet double - including a decisive 90th-minute goal - to book his side a spot in the League One play-off final against Yorkshire rivals Doncaster Rovers at the expense of heartbroken Carlisle.

Howson had hauled Leeds - beaten 2-1 in the first leg at Elland Road - level 2-2 on aggregate with the opener when he drove in a shot after 10 minutes following a pull-back from Dougie Freedman to stun home specialists Carlisle.

Leeds took a stranglehold on the game as the goal proved a body blow which took the wind out of the Cumbrians.

But United had to wait until the death for their winner, just when extra-time beckoned, with Howson firing a shot into the corner of the net.

4. 2005-06 Huddersfield Town 1 Barnsley 3, Barnsley win 3-2 on aggregate.

5. 2002-03 Sheffield United 4 Nottingham Forest 3, Blades win 5-4 on aggregate.

One of the greatest ever play-off second legs saw Neil Warnock’s Blades triumph in a seven-goal thriller with Forest at a spell-bound Bramall Lane - following a magnificent comeback.

David Johnson put Forest ahead after a mistake from John Curtis and Andy Reid’s volley added a second for the visitors just before the hour mark - and it looked to be all over for the Blades, who drew 1-1 in the first leg at the City Ground.

But the Blades showed wonderful resilience and character and free-scoring midfielder Michael Brown pulled one back with a deflected effort off Des Walker.

Then, interval substitute Steve Kabba scored a sweet volley to make it 2-2.

There was no away goals rule to come into force and the tighly contested tie went into extra-time.

Amid a cauldron of noise from home fans, a magical Paul Peschisolido effort and a Walker own goal put the home side 4-2 up on the night - only for Robert Page’s own goal at the other end to give Forest hope with a late goal.

But United held on for the win and a final place against Wolves at the Millennium Stadium.
